I will stay and live at Camp Hope for the entire duration of the 7-week internship program. I will participate in all Camp Hope activities, including all camps and discipleship activities. All the information provided in this form is true and correct to the best of my knowledge. I understand that providing false information may lead to immediate termination of my internship.

Camp Hope reserves the right to select candidates based on its internal criteria and decisions are final.
